Carnaval Hot Chocolate Permission Forms

Good day,
please find a copy of the Hot Chocolate Permission form for your child that is one of our Carnaval d’hiver stations.
Please ensure that your child returns the form no later than Tuesday, February 21st or we will assume that they do not have permission to receive hot chocolate or a granola bar.
